About the role

The role of a Support Worker is to care, support, and protect residents to enable them to lead a meaningful life. At MHC, we believe each resident is a human being with an equal right to life and good treatment as everybody else. Through the implementation of a person centred support plan, the Support Worker will strive to help people remain as independent as possible and to maximise their quality of life. They will work as part of a dedicated, qualified and capable staff team who put the people living here at the heart of what we do.

About the Service: Avalon is a community facing home, supporting for up to five people with a Learning disability and may have mental health problems. The home accommodates younger adults of mixed gender.  Avalon provides a homely environment which residents can consider to be their home and are encouraged to take an active role, and have their say in the daily running of the home. Staff promote and support residents with daily living skills.

This involves guidance and encouragement to partake in household tasks, learning basic culinary skills, laundry and other domestic duties. This is to promote a better quality of life and more independence. Support people to take an active role in the community by accessing local facilities and wider community resources in the spirit of social inclusion. Avalon is served well by local public transport. Buses operate a regular service between to neighbouring towns and the A55 expressway provides network links within North Wales.

Offering community based living in a homely environment in Kinmel Bay, Conwy, Avalon provides medium to long-term care for up to five men or women aged 18+ who have a diagnosis of a learning disability. Individuals may have coexisting secondary conditions including complex and/or mental health needs.

The step down service operates structured programmes and residents are encouraged to take an active role and have their say in the daily running of the home.
